Position Description:

The Mental Health Registered Nurse (RN) provides direct service to selected patients and families where there are complex emotional and/or psychiatric problems. Following assessment and establishment of a plan, these patients may be carried partially or totally by the Mental Health Nurse. The Mental Health Nurse will assist the health team with concepts in mental health care, will serve on the behavioral health team and serve as a resource for all staff.

The Mental Health Nurse will also carry a regular home care caseload as a Case Manager. The Nurse Case Manager is responsible for independent management of a diverse patient population requiring the use of advanced assessment, teaching and decision-making skills. The nurse is responsible for ensuring that appropriate referrals to other services are made, interdisciplinary conferencing takes place regularly, and appropriate documentation is completed. The Nurse Case Manager cares for a caseload of home health patients by evaluating the patient for appropriateness of home health and developing a patient centered plan of care in conjunction with the physician and interdisciplinary team. The RN provides follow up by evaluating the effectiveness of the home care plan and monitoring patient/family's response to the plan. The Licensed Practical Nurse (LPN) is a nurse that works under the direct supervision of an RN and performs all assigned tasks that are delegated from the RN. The Nurse educates patients, families, and caregivers to safely perform care in the home. The Nurse has knowledge of home care regulations and third-party reimbursement as it impacts the delivery of services. The Nurse must adhere to their scope of practice and follow all agency administrative, clinical and personnel policies and procedures. This position will have weekend and on-call responsibilities and is under the general supervision of a Team Manager.

Educational Requirements:

• RN Degree/NH/ME
• Two years of mental health experience preferred.
• Home Care experience minimum of one year preferred.

• Proficiency with medical EMR systems.
• Valid driver's license in good standing and vehicle insurance are required.
• Ability to provide trusted, compassionate and expert care.
