Qualifications:

• Bachelor's degree in Nursing, Healthcare Administration, or related field (preferred)

• 2+ years of experience in healthcare intake, referral management, or clinical operations

• Strong organizational, communication, and interpersonal skills

• Ability to work under general direction with sound judgment and independent decision-making

• Comfortable with technology and data analytics platforms (e.g., EMR, HCHB, SHP)

• A collaborative, solutions-oriented mindset with a commitment to patient-centered care
