Job Description:

The Hospice Manager is responsible for the coordination and collaboration of services within the hospice interdisciplinary team. This role ensures team cohesiveness, coordination of patient care, and evaluation of assigned team members. The Hospice Manager reports directly to the Hospice Director.

Key Responsibility:

• Staff Supervision:

  • Supervise and evaluate hospice staff.
  • Conduct competency assessments with the Hospice Quality Coordinator.
  • Monitor team productivity and ensure accurate time logs.
  • Perform hospice audits and promote timely documentation.
  • Hold regular staff meetings and foster a positive work environment.

• Quality Assurance:

  • Utilize data to enhance hospice programs.
  • Ensure adequate staff coverage and support quality initiatives.
  • Adhere to clinical practice standards.

• Team Development:

  • Manage patient distribution and triage calls.
  • Coordinate referrals and support new team member orientations.
  • Resolve interpersonal conflicts and utilize resources effectively.

• Communication:

  • Communicate staffing needs and issues to the Hospice Director.
  • Coordinate patient services and interdisciplinary collaboration.
  • Pursue professional growth and implement performance improvement strategies.
  • Complete required training and continuing education.

Qualifications:

• Graduate of an accredited nursing school.
• Current NH Registered Nurse (RN) license.
• Bachelor's degree or equivalent experience preferred.
• Two years of hospice experience preferred.
• Certified Hospice and Palliative Nurse (CHPN) preferred.
• Minimum of one year of supervisory experience.
• Strong computer skills.
