Job Description:

  • Responsible for the coordination of patient referrals; ensuring collaboration among referral sources and Hospice triage enhancing communication.
  • Offers education to the medical and lay community regarding Hospice & Palliative Care (PC) services.
  • Organizes regular visits to referral sources located in Strafford, Rockingham, Carroll & Belknap counties in NH as well as York County in Maine.
  • Communicates timely to Hospice/PC Director; problems, concerns or issues.
  • Identifies new referral sources that will benefit from liaison visits.
  • Develops positive relationships with referral sources/facility staff.
  • Provide education and support to Hospice Case Manager assigned to facility patients.
  • Promotes effective written/verbal communication.
  • Identify the needs of each facility and create work flow to support those needs.
  • Create packets/handouts with services specific to each facility.
  • Works closely with Advancement Director as it relates to referral sources.
  • Completes Hospice informationals as requested.
  • Facilitate/support CMs in implementing care plan meetings.
  • Attend Care Plan meetings as needed or requested.
  • Use collaborative approach in medication management such as comfort kit protocols and standing orders.
  • Provide ongoing support to both hospice and facility staff.
  • Act as a mediator between all hospice disciplines and facility staff with a goal of maintaining open communication and best practice.
  • Work with Hospice Director on program growth and collaborative practice initiatives with facilities.
  • Assist in coverage for hospice nurse liaison during vacations as needed.

Qualifications:

  • Completes work and documentation with accuracy and within agency time frames.
  • Requires minimal supervision and is self-directed.
  • Protects and honors customer and coworker confidentiality.
  • Meets attendance and punctuality expectations.
  • Demonstrates cooperation with scheduling requests to meet agency needs.
  • Consistently adheres to agency dress code.
  • Demonstrates knowledge of policies and procedures applicable to own job position. Adheres to agency policies and procedures.
  • Maintains agency property, supplies and equipment in manner that demonstrates accountability.
  • Works towards resolution of interpersonal conflicts as they arise.
  • Develops cooperation and collaborative work efforts that generally benefit all employees.
  • Demonstrates a desire to set and meet objectives and to find increasingly efficient ways to perform tasks.
  • Performs other duties as assigned.
  • Demonstrates a strong knowledge of Hospice regulations and philosophy.
